Exceptional Quality Maker Marks Question

I was just curious the Lady or Lord, that is seen before a maker's mark for exceptional quality crafted items, is because of their fame (i.e Glorious Lord) or because the player has manually named their character "Lord or Lady" before the rest of their character name?

Crafters gain fame by filling and handing in BoDs, eventually raising them to Lord or Lady.

Could be both, but usually fame.
