1 and 2 happened to me a few times. No other players around. Triggered by me getting overwhelmed and running away, Pirate would focus on me and follow at mount speed. I took one from a back spawn point to the front spawn point.

The times we used them we were near an island. So we thought they were working. That is a fun way to kill the orcs. You could go in 2 teams and kill both sides at the same time.

Void Pool is fun for killing. Do what you like to do fast for Hats. 74 today doing beacons and ships. I have a potion from December that I have somehow not accidently clicked in my backpack.

I turned a Bone helm into a Dragon Helm and it still gives me 3 LMC extra. Maybe it just has to be the same basic type of material and it takes the original items LMC properties.
