I just tried to repro this and was unable. After stacking 10 keys up to 100 I was able to successfully insure/uninsure via targeted and menu insurance. Could you provide more information? Also, the stacking only applies to non-master keys.

Spawned is meant to mean once the chest is spawned into the world. It is spawned when you start digging. You can see it underground by viewing object handles.

Once the chest is spawned, so if you switch after you see the chest your luck has already been factored in.
